I agree to have my child participate in the following activity or function: Preschool Pals and Page under the supervision of, or sponsored by Keiki Reading Nest on the following days: Mondays or Fridays afternoons.

I further agree to assume responsibility for any liability that may arise out of ordinary negligence or otherwise, with the exception of acts of gross negligence, or intentional, willful or wanton misconduct.

I fully understand and acknowledge that risks and dangers are inherent in the aforementioned activity and that said child’s participation in such activities and/or use of the accompanying equipment may result in injury or illness or death or damage to personal property. On behalf of myself, my heirs, assigns, child, and next of kin, I waive all claims for damages, injuries sustained to my child that I may have against Keiki Reading Nest arising out of ordinary negligence or otherwise with the exception of acts of gross negligence as indicated above.

I warrant that the aforementioned child is in good health and possesses the requisite skills and/or abilities to participate in said activity.

 

Photo/Media Release: 

I hereby understand that Keiki Reading Nest from time to time may use photograph, video, or otherwise record classes or events occurring during schedule programs and place such photographs and videos on its Website or social media platforms.  I hereby consent to the use of my images that may appear in any such photographs or videos.  If I would not like these photos to be posted or used for any promotion, please advise Keiki Reading Nest in advance.
